Information on this public website is provided for general educational and informational purposes. Reading the website, sending an ordinary email, using the scheduling widget, or participating in a discovery call does not by itself establish a nurse-client, clinician-patient, or doctor-patient relationship and is not a substitute for individualized medical advice.
Amy's RN wellness scope
Amy may provide holistic and lifestyle-based nursing assessments, wellness education, coaching, health-promotion services, and individualized Wellness Plans within her authorized RN scope. The website should not be interpreted as expanding her legal authority or as representing that she independently provides medical diagnosis, treatment, prescribing, or other advanced-practice functions.
Medical collaboration and Nurse Practitioner services
When medical evaluation, diagnosis, prescribing, treatment, medical orders, or laboratory ordering are appropriate and available through Ohr Wellness, those functions are performed by an appropriately licensed Nurse Practitioner (NP) or other qualified medical provider in accordance with applicable law and the provider's own scope. Availability varies by state and service.
Laboratory services
Laboratory testing, when offered, is ordered through an appropriately licensed NP or other authorized medical provider in accordance with applicable laws and regulations. Amy may use laboratory information for nursing education, wellness planning, and collaboration within her authorized scope. Laboratory information is not used by Amy to independently diagnose disease or prescribe medical treatment.
FDA and educational disclaimer
These statements have not been evaluated by the Food and Drug Administration. Information provided by Ohr Wellness, including information about nutrition, supplements, herbs, lifestyle, and Wellness Plans, is not intended to diagnose, treat, cure, or prevent any disease. It is provided for educational and wellness purposes. Consult your physician, Nurse Practitioner, pharmacist, or other appropriately licensed medical provider before starting, stopping, or materially changing medications, supplements, exercise, diet, or other parts of your healthcare regimen.
Wellness Plan disclaimer and assumption of risk
If you choose to use an Ohr Wellness Wellness Plan, you acknowledge that participation in fitness, movement, nutrition, lifestyle, supplement, or other wellness activities may involve inherent risks, including personal injury or health-related issues. You are responsible for deciding whether a suggested activity is appropriate for you and for seeking medical clearance when appropriate.
Participation in a Wellness Plan is voluntary. Choosing not to use a Wellness Plan or a particular wellness resource does not result in punishment or adverse action. Use of a Wellness Plan does not establish a doctor-patient relationship with Amy.
To the extent permitted by applicable law and any signed agreement, users of a Wellness Plan accept responsibility for voluntary participation and the ordinary risks associated with wellness activities. Any contractual waiver or release of liability is presented separately for affirmative acknowledgment during secure onboarding; this public webpage is not intended to replace a signed consent or waiver.
Accuracy, omissions, and no guarantees
Ohr Wellness strives for accuracy but does not warrant that public educational content or Wellness Plan materials are free from every error, omission, or outdated statement. Health and wellness outcomes vary. No testimonial, example, educational statement, or wellness recommendation is a promise or guarantee that another person will achieve the same or similar result.
Nutritional supplements and herbs
Supplements and herbs can have physiologic effects and may interact with prescription medications, over-the-counter products, medical conditions, pregnancy, or other factors. Keep all of your healthcare providers informed about the products you use and discuss any supplement regimen with an appropriately licensed medical provider before implementation.
Virtual services and geographic availability
Ohr Wellness is a fully virtual practice. Services are available only where the requested service may legally be provided based on the client's physical location, Amy's nursing authority, the collaborating provider's authority when applicable, and state-specific law. Scheduling does not guarantee eligibility.
Pediatric services
Pediatric wellness services are educational and supportive and do not replace the child's pediatrician or specialists. Children with symptoms, diagnosed conditions, medication needs, urgent concerns, or complex medical issues should remain under appropriate medical care.
No emergency care
Ohr Wellness does not provide emergency services through this website, ordinary email, Practice Better scheduling, Wellness Plans, or discovery calls. Call 911 or seek immediate emergency care for urgent or life-threatening concerns.
Insurance and payment
Availability of insurance billing does not guarantee coverage or reimbursement. Coverage depends on the service, licensed provider, payer requirements, credentialing, jurisdiction, benefits, and other plan terms. Self-pay individuals may have rights to a Good Faith Estimate when applicable.
